Church-wide
annual events
These events have the potential to affect the entire church congregation and are part of Lone Star Cowboy Church annually.
The request time for a Church-wide Annual event is 6 months due to the level of planning, prep, and coordination of all departments.
A Church-wide Annual event includes ALL the options for support requests, including
in-service promo and/or recap videos. Examples of these events are Easter, Christmas Eve, Big Band, One Connection, Night of Worship, Rodeo Bible Camp, One Day to Feed the World, and Cowboys for Kids.
Major Ministry specific events
These events are hosted and coordinated by individual ministries, but require a significant amount of planning, preparation, and support.
The request time for a Major Ministry Specific Event is 6 months, and includes all the support options, including pre-service promo videos. However, some top-level items, like in-service
videos are on approval only.
Examples of this would be Camps and Retreats, Mission
Trips, Study Trips, Volunteer Celebration, He Paid Your Fees, Night of Worship, Family
Fellowships, VBS, 3Cross Gala, Mother/Son and Ladies Night Out.
Minor Ministry specific events
These events require reasonable planning and preparation. The request time for a
Minor Ministry Specific Event is 2 months, and support is limited to:
Digital slide, signage/posters, social media promo, web event listing, sign-up form, lighting | audio | presentation content, tv cart (no in-service videos)
Examples of this include but are not limited to monthly arena events, Local Missions, GrowthTrack, student back to school event or bible studies.

Once your event request form has been submitted and approved,
please submit a Creative Calendar Request. It will then be up to you to schedule a
meeting with the Creative Department. We will go over each line item to make sure we
understand every detail of your event. Please be prepared to bring any details or ideas
that would help us fulfill your request.
Please use this form to request approval for your events. Upon approval, you can then submit requests to our support departments for items you need to successfully host your event. Vision Request Major must be submitted 6 months in advance of your event date. Vision Request Minor event requests must be submitted 4 months in advance of your event date. Please note: after receiving approval for your event, you must then complete any/all other request forms for facilities and creative needs.

General Church Forms
Facilities Calendar WILL NOT be changed until the HARD COPY is delivered to Linda Dorsey.
Please submit one request for each calendar event change.
Use this form to report accidents, injuries, medical situations, criminal activities, traffic incidents, or student behavior incidents. If possible, a report should be completed within 24 hours of the event. Immediately notify a senior LSCC Staff member when an incident occurs.
Please use this form to request any facilities or maintenance needs. Please give as much detail as possible to help our maintenance department meet your needs. For immediate repairs, please contact the maintenance dept. directly.
Please use this form to request facilities and creative support for any funeral hosted here at Lone Star.

Production Form
Please use this form when requesting Audio,
Video & Lighting .
Please give as much detail as possible to help our Production Department meet your needs.
This includes: audio, lighting and media (including ProPresenter operation, Live Streaming or media carts).
Also include on this request form is info requesting files to be uploaded. Please note that this does not replace the Creative Request for events.
